RESUMO

As summer slowly gives way to autumn, I find myself reflecting on the activities of the summer. One memory that stands out is attendance at the 29th International Congress of Nurses held in Montreal in July 2023. Cohosted by the International Council of Nurses and the Canadian Nurses Association, it was an amazing opportunity to meet and learn with nurses from all over the world. It was wonderful to be able to get together in person and feel the palpable energy created by more than 6,000 delegates from 123 countries gathered together for a common purpose. Background: During the past 50 years, numerous conceptual frameworks have been used to describe and evaluate advanced practice nursing (APN) roles and outcomes. The International Council of Nurses (ICN) released an updated description of APN characteristics intersecting with currently utilized frameworks allows for assessing relevance to practice. Purpose: The review aimed to examine the alignment of established and commonly used conceptual frameworks related to advanced practice roles with the ICN APN guidelines characteristics. This will help identify a globally relevant framework for APN roles. Method: PubMed, Cumulative Index in Nursing and Allied Health Literature, and ProQuest Central databases were searched using terms that characterize APN and conceptual frameworks. To address currency and relevance of frameworks was applied to discovered frameworks identifying and reviewing in detail those which were cited more than 15 times during the last five years. Results: This search found over 1107 publications. Of these, nine conceptual frameworks met all inclusion criteria. The frameworks captured some of the characteristics described by the ICN. Eight addressed all categories but in a limited fashion. Implications for Practice: Reviewing recently and frequently cited frameworks can inform the applicability in the interested readers, own nursing practice. This review not only does this but also examines the global uptake and the relationship to international standards of APN to provide additional information on the frameworks' reach and worldwide transferability. However, further research examining the relevance of these frameworks in low- and lower-middle-income countries is needed to understand their importance.

AIM: The aim of this article is to explore the role of nurses with regards to climate change and other humanitarian challenges. BACKGROUND: The climate crisis facing our planet is not just about extreme weather events. To be properly understood and addressed, climate change should be recognised as a global public health emergency that needs urgent attention from governments everywhere. DISCUSSION: In this article, International Council of Nurses (ICN) Chief Executive Officer Howard Catton discusses the links between climate change, nursing practice and advocacy. He argues that the crises the world faces, including those related to global warming, can be handled better if they are understood as public health crises, and that having nurses involved in policymaking at every level in society provides the best chance of individuals' health needs being met. Conclusion and Implications for Nursing and/or Health Policy: Nurses must be actively involved in policymaking that addresses the humanitarian and public health impact of climate change and natural and human-made disasters. Only half the world's countries have a government-level chief nursing officer who is able to represent the nursing profession at the highest levels of policymaking. This deficit should be corrected as soon as possible so that the voices of nurses are heard and respected, and patients everywhere can benefit from the direct input of nursing into all government policies that influence people's health and well-being.

Nursing is a global profession, and the COVID-19 pandemic has illustrated just how interconnected and interdependent nursing and health systems are across jurisdictions. The International Council of Nurses (ICN) is a federation of more than 130 national nursing associations and serves as a key policy voice at the global level. Every two years, their congress brings together thousands of nurses and stakeholders to share and disseminate knowledge. Although Canadian presence has historically been strong in these global fora, there is a lack of literature that focuses on discussing the implications of these global discussions on Canadian nurses' policy and advocacy engagement. In this article, we provide a framework of key policy themes as observed during the ICN's 2021 virtual congress. We discuss how these global policy themes align with Canadian nursing, health and public policy priorities and conclude with questions to guide nurses in embedding a global perspective into their research, policy, education and practice initiatives.

Every year on International Nurses Day (IND), the International Council of Nurses (ICN) releases a report with resources that can be used by nurses around the world. This year's report responds to the World Health Organization's (WHO) latest strategy for nursing and midwifery, and the findings of the State of the World's Nursing report. In this article, ICN Chief Executive Officer Howard Catton describes the main points of the IND report and recommends that nurses everywhere read it, alongside the WHO strategy, and use that information to increase their knowledge of the global nursing workforce and influence global healthcare policy. It is a toolkit that can assist nurses, other healthcare providers, governments and international organisations to turn global strategy into meaningful local action and improved clinical practice on the ground.

ABSTRACT Objective: to reflect on the equivalence between the concepts of the International Classification for Nursing Practice and the Systematized Nomenclature of Medicine International - Clinical Terms. Method: theoretical reflection based on the analysis of equivalence between the concepts of diagnoses, results and nursing interventions of the International Classification for Nursing Practice and the hierarchy of the Systematized Nomenclature of Medicine International - Clinical Terms. The researchers' experience and articles on the subject provided support for analysis. Results: nursing diagnoses and results of the International Classification for Nursing Practice are present in the hierarchies "clinical finding", "disorder" and "problem situation", while the interventions are included in the hierarchies "procedure" and "regime/therapy". The main causes of non-equivalence are linked to the problems of the specificity of the concept. Cross-mapping will require analysis by nursing specialists to improve the representativeness of the concepts. The equivalence table must be translated into Brazilian Portuguese, but the entire Systematized Nomenclature of Medicine International - Clinical Terms lacks interdisciplinary work. Conclusion: the representation of the International Classification for Nursing Practice in systematized Nomenclature of Medicine International - Clinical Terms will bring benefits related to the clarity of concepts. The concepts of nursing classification that are not equivalent will require conceptual analysis. The lack of translation of the Systematized Nomenclature of Medicine International - Clinical Terms for the Portuguese language will reflect the development of terminological subsets of the International Classification for Nursing Practice.


RESUMEN Objetivo: reflexionar sobre la equivalencia entre los conceptos de la Clasificación Internacional para la Práctica de Enfermería y la Nomenclatura Sistematizada de Medicina Internacional - Términos Clínicos. Método: reflexión teórica basada en el análisis de la equivalencia entre los conceptos de diagnósticos, resultados e intervenciones de enfermería de la Clasificación Internacional para la Práctica de Enfermería y la jerarquía de la Nomenclatura Sistematizada de Medicina Internacional - Términos Clínicos. La experiencia de los investigadores y los artículos sobre el tema sirvieron de apoyo para el análisis. Resultados: los diagnósticos y resultados de enfermería de la Clasificación Internacional para la Práctica de Enfermería están presentes en las jerarquías "hallazgo clínico", "trastorno" y "situación-problema", mientras que las intervenciones están incluidas en las jerarquías "procedimiento" y "régimen/terapia". Las principales causas de la no equivalencia están vinculadas a los problemas de especificidad del concepto. El mapeo cruzado requerirá el análisis de expertos en enfermería para mejorar la representatividad de los conceptos. La tabla de equivalencia debe ser traducida al portugués brasileño, pero la totalidad de la Nomenclatura Sistematizada de Medicina Internacional - Términos Clínicos carece de trabajo interdisciplinario. Conclusión: la representación de la Clasificación Internacional para la Práctica de Enfermería en la Nomenclatura Sistematizada de Medicina Internacional - Términos Clínicos traerá beneficios relacionados con la claridad de conceptos. Los conceptos de clasificación de enfermería que no sean equivalentes requerirán un análisis conceptual. La falta de traducción de la Nomenclatura Sistematizada de Medicina Internacional - Términos Clínicos para el portugués se reflejará en el desarrollo de subconjuntos terminológicos de la Clasificación Internacional para la Práctica de Enfermería.

A tabela de equivalência deverá ser traduzida para o português brasileiro, porém a totalidade da Systematized Nomenclature of Medicine International - Clinical Terms carece de trabalho interdisciplinar. Conclusão: a representação da Classificação Internacional para a Prática de Enfermagem na Systematized Nomenclature of Medicine International - Clinical Terms trará benefícios relacionados à clareza dos conceitos. Os conceitos da classificação de enfermagem que não foram equivalentes necessitarão de análise conceitual. A ausência de tradução da Systematized Nomenclature of Medicine International - Clinical Terms para o português refletirá no desenvolvimento de subconjuntos terminológicos da Classificação Internacional para a Prática de Enfermagem.

RESUMO

Annette Kennedy's four year term as President of the International Council of Nurses ended during the organisation's virtual Congress in November. Here she writes about her time in office and the state of the world's nursing after 2 years of the pandemic. She acknowledges the strains that COVID-19 has put on nursing but affirms that nurses and nursing have come through the pandemic as a stronger, more influential and more cohesive profession.

BACKGROUND: In 2020, the SARS-CoV-2 pandemic forced the redesign of the International Council of Nurses' (ICN) Global Nursing Leadership Institute (GNLI) that heretofore had included an intensive, week-long residential experience in Geneva, Switzerland. AIM: This paper describes the development, implementation, challenges and preliminary outcomes of an approach that regionalized GNLI and delivered it online, while preserving some cross-regional connections among participants. RESULTS: A virtual approach to a global policy leadership programme cannot replace the richness of an intensive, concentrated residential one, but the redesign of GNLI has given ICN, its member national nursing associations and individual nurse leaders previously unexplored opportunities for networking and collaboration aligned with the six WHO regions, and for cross-region networking and discussions of global issues. CONCLUSION AND IMPLICATIONS FOR NURSING AND/OR HEALTH POLICY: The SARS-CoV-2 pandemic has highlighted the need for nursing leadership in policy and practice. A virtual approach to leadership development can increase the reach of programmes that seek to build nursing's capacity to advance health-promoting policies in workplaces, organizations and nations.

The International Council of Nurses is in a unique position to represent nurses at the World Health Organization, and its task has never been more urgent than this year. Amid the global COVID-19 pandemic, the death rates of nurses and other health care workers are truly shocking, with around 115 000 deaths. However, many countries do not collect statistics on health and care workers' deaths and infections from COVID-19, so the full extent of this awful situation is not known. At this year's World Health Assembly, the body that sets the World Health Organization's agenda for the following year, the International Council of Nurses' 50-strong delegation voiced the concerns of nurses and ensured that the views and advice of nurses were heard by the World Health Organization's 194 member states' delegates. Here, the International Council of Nurses' Chief Executive Officer Howard Catton describes how the Council is influencing health and care policy worldwide. He urges nurses everywhere to hold their politicians to account.

Es tiempo de considerar las oportunidades que actualmente se presentan a enfermeras/os, a raíz de contar con el respaldo legal que define el campo del ejercicio profesional(1-2); disponer de un corpus deontológico consolidado en principios y valores, así como, una definición de las competencias para la gestión del cuidado, para cumplir a cabalidad el compromiso de la enfermería con la sociedad, desde una práctica ética y autónoma para tomar decisiones respecto al cuidado(3). En los lineamientos del Consejo Internacional de Enfermeras (CIE), se propone que para el desarrollo del rol sociopolítico, a través de funciones esenciales, que junto con los cuidados autónomos y de colaboración, se agreguen la defensa y el fomento de un entorno seguro, la gestión del cuidado y de los sistemas de salud, el desarrollo de investigaciones, la participación en la formación y en las políticas sanitarias(4). Las/los enfermeras/os muestran diversos niveles de autonomía, se observa una mayor independencia en las funciones derivadas de la asistencia, especialmente en lo referente a educación, promoción en salud y en la realización de procedimientos técnicos, en contrapartida con un menor grado de independencia en las funciones derivadas de la gestión e investigación. Esta situación se ve agravada por la escasa participación de los profesionales en las organizaciones gremiales, lo cual limita una defensa colectiva para el reconocimiento social del rol integrado(5). Para potenciar el cumplimiento de manera integral de las funciones, es necesario fortalecer desde la formación de enfermeras/os la sensibilidad social, el liderazgo, el compromiso político y una comunicación horizontal, a través de un paradigma transformador, que se proyecte más allá de ser un apoyo a la comunidad a resolver sus problemas socio-sanitarios, sino que además, brindarles las herramientas necesarias para el empoderamiento social, a través de un auténtico diálogo social que les permita tomar sus propias decisiones y que se reconozcan como ciudadanos responsables del cumplimiento de sus deberes, conscientes de sus propios derechos en el área sociosanitaria(6). La enfermería desde sus inicios se ha movilizado en escenarios duales de invisibilización-reconocimiento, sumisión-autonomía, enfermería vocacional femenina-enfermería profesional científica, situaciones que, a su vez, generan un significado paradojal para el colectivo, insatisfacción-enriquecimiento del rol profesional.

El empoderamiento del rol sociopolítico plantea grandes desafíos al profesional de enfermería, como ciudadanos comprometidos con las personas, familias y comunidades, considerando los requerimientos del escenario socio-sanitario actual. Para asumir este rol en propiedad se deben superar limitaciones tanto internas, como externas. El objetivo de esta reflexión es conceptualizar el rol sociopolítico de enfermeras/os e identificar las posibles barreras existentes para asumir dicho rol. Finalmente, se realizará una propuesta para configurar y potenciar una nueva enfermería que evidencie autonomía y liderazgo para contribuir a la formulación de políticas sanitarias, que dé cuenta de los desafíos planteados por el actual escenario socio-sanitario.


The empowerment of the socio-political role poses great challenges to the nursing professional, as citizens committed to people, families and communities, considering the requirements of socio-sanitary context. To assume this role in property, both internal and external limitations. The objective of this reflection is to dimension the sociopolitical role of nurses and identify the possible barriers. Finally, a proposal will be made to configure and promote a new nursing that shows autonomy and leadership to contribute to the health politics, showing the necessary changes of socio-sanitary context.

The impressive work of nurses, midwives and other health and emergency workers is far from over in the COVID-19 pandemic. But opportunities to learn from adversity are being explored and enacted in many settings and locations. Many ideas, opinions, research findings, innovations in practice, and policy recommendations are described in this Special Issue of International Nursing Review, The Official Journal of the International Council of Nurses in Switzerland. The contents will help inform educators, policymakers, health reformers, researchers, governments, leaders and practitioners about nursing's future. On International Nurses Day 2021, we dedicate this issue to the memory of the yet unknown number of nurses and midwives around the world who have died during this pandemic.
